Overview
Patient notes in Reflective are private and visible only to you, the therapist, and any colleagues ("My Team") in your practice if you are on the Practice plan. These notes do not appear in the patient’s view. They are a convenient way to document session details, jot down reminders, and keep track of important information.
How to Add a Note
To add a new note, open Patients from the main navigation. Select the patient whose profile you want to update, then choose the Notes tab. Click Add Note to create a new entry.

You can adjust the date and time if you want to backdate or otherwise specify when the note was taken, which can be helpful if you’re logging a session that happened earlier. If a category is relevant to your clinical workflow, you can select it before entering your text in the note field. Notes are listed chronologically according to the date/time you designate.
Hit "Save" to save the note to the profile.

Team Visibility
If you are on the Reflective Practice plan for teams, these notes are also visible to other therapists or team members within your organization who have access to this patient. This shared visibility can help your practice maintain continuity of care when multiple professionals collaborate on a patient’s treatment.
Email note to Admin
If you are on the Practice plan, you can also "Email note to Admin" to let your team's Admin know important information. This can be helpful if you want help transition a client, let them know about a problem, or anything else. Just switch the toggle to "Yes" to include the note as an email directly to your Practice Admin.

Did You Know?
You can also add notes to a group profile and automatically email other group facilitators, ensuring everyone stays informed. This can be especially useful for group therapy settings.
